Form B (fire) format: the prescribed Maharashtra certificate, field by field

Updated By Flamekite Editorial DeskFacts checked against official sources on

Quick answer

The Form B format is printed in the Maharashtra Fire Prevention and Life Safety Measures Rules, 2009 (gazette of 23 June 2009): a one-page certificate in which a Licensed Agency certifies that it inspected the building's fire prevention and life safety measures and that they were maintained in good repair and efficient condition during a stated period. The agency fills 6 blanks, signs with its address and licence number, and appends its inspection report.

Since 7 July 2026, MFS circular MFS-2026/888 adds a covering-letter format for the owner, a maintenance and testing report and a site inspection checklist, and bars the Maharashtra Fire Service logo and any added or deleted word. MFS publishes no separate Form B download: the text is on page 18 of the Rules PDF, linked below.

How a Licensed Agency completes the Form B format in 6 steps

  1. Name the premises in full

Fill the two dotted lines after "namely*" with the building's name and its detailed location and postal address, as the form's footnote asks.

  2. Write the 6-month period

A January Form B states 01 July to 31 December of the previous year; a July Form B states 01 January to 30 June of the current year.

  3. Append the inspection report

The form certifies that the inspection details are in "the report appended herewith". Attach the Appendix for fire fighting, detection and alarm, and passive systems, whichever the agency maintains.

  4. Choose a qualified signatory

Since 12 June 2025 the person who signs must hold a rule 12 qualification: a fire engineering qualification, or an engineering degree or diploma with at least 3 years' experience.

  5. Sign digitally and add the licence number

Use the digital signature that appears on the agency's licence from the Director, and the licence number as printed on the MFS list.

  6. Add place and date, then hand over

Date it in January or July and give it to the owner with the testing report and the site inspection report, for the owner's covering letter.

Above the text sit the heading "FORM B", the line "[See section 3(3) and rule 4(2)]", the title naming the January and July certificate, and the word "CERTIFICATE". Below it: "Place" and "Date" on the left, "Signature and Address of the Licensed Agency" and "Licence No." on the right, and one footnote: "Detailed location and postal addresses are to be provided here". Nothing else is printed on the page.

Every blank on the Form B format, and what goes in it

The agency fills 6 blanks and attaches 1 report. Each row gives the printed words, what the agency writes there and the rule or circular that governs it.

Flamekite counts 6 blanks: premises, period, place, date, signature and address, licence number. The form has no box for the owner's signature: the owner signs the covering letter instead.

What the 2025 amendment changed in the Form B wording

The Amendment Rules of 12 June 2025 rewrote rule 4(2) but did not reprint Form B, so the rule and the form now read slightly differently.

As printed in 2009, rule 4(2) asked for a certificate "twice a year in the months of January and July" that the fire measures were "in good repair and efficient condition as specified in sub-section (1) of section 3 of the Act". The 2025 amendment adds "and the National Building Code and Indian Standard Code" after the reference to the Act, and "in preceding six months" after "efficient condition".

Form B itself was not substituted. It still certifies upkeep "as required under the provisions of" the 2006 Act alone, and still leaves the period as a blank. In practice the 2026 guidelines close both gaps: clause 5 lists 14 Indian Standards the agency maintains to, and clause 2 fixes the 6-month period.

The 2025 Rules also added rule 14(4): Forms A, B, A-1 and B-1 must be issued under the signature of a person holding the rule 12 qualification. Rule 4(5) lets the owner submit any of them in person, by registered post, by courier or online.

One difference is easy to miss. Forms A, A-1 and B-1 each end with a note that every building needs a separate certificate. Form B has no such note; its only footnote asks for the detailed location and postal address. Clause 6.1 of the 2026 guidelines fills the gap by requiring one agency per system group for the entire building or occupancy.

Form B-1, inserted after Form B in 2025, is headed "[See rule 4(5)]" although rule 4(4) is the rule that creates it. Flamekite records the heading as printed.

The Form B covering letter format, line by line

Clause 7 of the 2026 guidelines prescribes the letter the owner or occupier writes on its letterhead. Flamekite splits the format into its 12 parts, in the order printed.

  • Reference number and date, written "No. / Date: Jan / July". MFS Form B guidelines
  • Addressed to the Director, Maharashtra Fire and Emergency Services, Hans Bhugra Marg, Kalina, Mumbai 400 098, or to the Chief Fire Officer with the name and address of the local fire and emergency service. MFS Form B guidelines

The Form B site inspection report format: 5 system checks

Clause 9 of the guidelines prints a "System Check List" that the agency and the owner sign. It opens with 3 general items, runs through 5 system parts, A to E, and ends with a Form B verdict.

  1. 01 to 03. The building

Type of occupancy, ticked from 10 boxes (residential, assembly, industrial, educational, business, storage, institutional, mercantile, hazardous, mixed); the building in-charge's name, phone and e-mail; the Final Fire NOC or OC with issuing authority, letter number and date. MFS Form B guidelines

  2. A. Fire extinguishers

Counts of ABC, CO2, foam, water CO2 and other types, then 3 yes-or-no questions: easily accessible, enough installed, properly maintained. MFS Form B guidelines

  3. B. Fire tanks and pumps

Underground and overhead tank capacities in litres, then 6 pumps (main hydrant, hydrant jockey, main sprinkler, sprinkler jockey, diesel engine, booster), each with mode, make, flow, head in metres, power in HP and whether a run test was done. MFS Form B guidelines

  4. C. Hydrant and sprinkler system

Test figures the Form B maintenance report must show

Clause 8 of the guidelines sets the maintenance and testing report under the Indian Standards. These are the figures a Form B's report is checked against.

How the Form B format changed, 2009 to 2026

The one-page form of 2009 is unchanged in words; 6 later instruments changed what goes around it.

  1. 23 June 2009

The 2009 Rules print Form B: a one-page certificate signed by the Licensed Agency with its address and licence number. Rules 2009

  2. 16 October 2015

What an agency may add to the Form B format, and what it may not

Clause 6 of the 2026 guidelines sets 3 things an agency may or must add, and 2 it may not. None of the gazette words may be added to or deleted.

  • Allowed

    The agency's letterhead

"To provide authentication of the Form B, Letter head may be used by License agency along with their digital signature on it." MFS Form B guidelines

  • Required

    The licence's digital signature

The owner and the local fire service must both ensure the Form B carries the same digital signature as the licence the Director issued. MFS Form B guidelines

  • Allowed

    A barcode

"License Agency may also use the Barcode system, if required." MFS Form B guidelines

  • Not allowed

    The Maharashtra Fire Service logo

"logo of Maharashtra Fire Service shall not be used on such certificate." A Form B is the agency's certificate, not the fire service's. MFS Form B guidelines

  • Not allowed

    Any added or deleted word

"There should not be any addition or deletion of any word (s) as given in the Form B." Extra lines such as a validity date or a fire NOC label fall foul of this. MFS Form B guidelines

  • Not asked

    Stamp paper or a notary

Neither rule 4, the printed Form B nor the 2026 guidelines ask for either. The Rs 100 stamp paper in Form M is the agency's own licence declaration, not part of Form B. MFS Form M

Form B format errors that get a certificate refused

Beyond the logo and wording rules, 7 format defects have a written rule against them.

  • If The signature is a scan, or a digital signature that differs from the licence

Since 15 February 2022 fire officers check that the Form B carries the same signature as the licence; get it re-signed with the licence's digital signature. MFS circular 236

  • If Signed by a person without the rule 12 qualification

Rule 14(4), in force since 12 June 2025, requires a signatory with a fire engineering qualification, or an engineering degree or diploma and 3 years' experience; have that person sign again. Amendment Rules 2025

  • If The period blank is empty, a single month or a whole year

Write the 6 months the form speaks for: 01 July to 31 December of the previous year, or 01 January to 30 June. MFS circular 1600

  • If The premises line gives only a building name

The form's footnote asks for the detailed location and postal address. Rules 2009

  • If The agency's licence ran out more than 3 months ago

The local fire service may accept a Form A or B only within 3 months of the licence's expiry; after that a currently licensed agency must issue it. MFS Form B guidelines

  • If The agency is not licensed for the system it certifies

MFS licenses fire fighting, detection and passive systems separately, each in classes A to D, on a separate application; check the class on the list against the Appendix. MFS Form M

  • If A "Form B" template from another state

Tripura's Fire Safety Rules, 2026 use "Form B" for a yearly agency certificate at renewal. Only the Maharashtra 2009 text is a Maharashtra Form B. Directorate of Fire and Emergency Services, Tripura

Form B format worked cases: 5 buildings, 5 forms

Flamekite's reading of the prescribed formats for 5 example buildings; the buildings are illustrations, the forms and rules are the states'.

  1. Case 1

A 12-storey housing society in Navi Mumbai, January 2027

One agency maintains the pumps, hydrants and sprinklers; another maintains the fire alarm.

What applies Each agency fills its own Form B for its system group with the period "01 July 2026 to 31 December 2026", appends its Appendix and site inspection report, and signs with its licence's digital signature. The society's secretary signs the covering letter, naming both agencies and enclosing both certificates. MFS Form B guidelines

  2. Case 2

    A 75 m residential tower in Pune

Above 70 m, so a section 45 building with an automated continuous monitoring system.

What applies Its agency uses Form B-1, not Form B, and reports the inspection against the 2025 Rules' Appendix: pump run status, tank levels, riser pressure and panel status. One Form B-1 per tower. Amendment Rules 2025

  3. Case 3

    A shopping mall in Lucknow

Holds a UP Fire Safety Certificate and a Fire Safety Officer.

What applies Each January and July the owner fills Form A-8 on the UP portal: certificate number and validity, the agency's licence numbers, the electrical and lift safety audit certificates and the officer's name. The qualified agency certifies the period beneath. UP Rules 2024

  4. Case 4

    A school building in Imphal

Educational, so it needs a Fire Safety Certificate under section 24(3).

What applies The owner signs Form C, stating the certificate number, the date the Fire Safety Officer checked the measures and that no alterations were made; the officer countersigns. Government of Manipur

  5. Case 5

Questions about the Form B format

Is there an official Form B format PDF?

Not as a separate file. On 25 October 2026 the Form B text was only in the Maharashtra Rules, 2009 PDF on mahafireservice.gov.in (page 18), and the covering letter, testing report and site inspection formats in the 7 July 2026 guidelines PDF. Flamekite links both and publishes no copy of its own.

What is written in a Form B fire certificate?

The Licensed Agency certifies that it inspected the fire prevention and life safety measures in a named building, that they were maintained in good repair and efficient conditions during a stated period under the 2006 Act, and that the inspection details are in the appended report. It ends with place, date, the agency's signature and address, and its licence number.

Can a Form B be printed on the agency's letterhead?

Yes. The 2026 guidelines allow the agency's letterhead with its digital signature, and a barcode. The Maharashtra Fire Service logo may not be used, and no word of the prescribed text may be added or deleted.

What period is written on a Form B?

A January Form B states 01 July to 31 December of the previous year; a July Form B states 01 January to 30 June of the same year. Since 2025 rule 4(2) says the certificate covers the preceding six months.

What is the format of the Form B covering letter?

Clause 7 of the 2026 guidelines: the owner's letterhead, dated January or July, to the Director or the Chief Fire Officer, subject "Submission of Form B", citing the Act, the Rules and the Final and Provisional approval numbers, naming the agency and the test date, listing 6 enclosures, with 4 declarations and acceptance of liability under section 36.

How is Form B-1 different from Form B?

Form B-1, added on 12 June 2025, is for section 45 buildings with an automated continuous monitoring system. It certifies maintenance with the help of that system, refers to the Rules' Appendix, and needs a separate certificate for each building.

Is Uttar Pradesh's Form A-8 the same as Form B?

No. Form A-8 is the owner's bi-annual declaration under UP's 2024 Rules, filed on the UP portal, with 6 declarations including electrical and lift safety audits and the Fire Safety Officer; the qualified agency certifies the maintenance period beneath it.

Does a Form B need stamp paper or a notary?

Neither rule 4, the printed Form B nor the 2026 guidelines ask for stamp paper or notarisation. The agency's digital signature, matching its licence, is what authenticates it.
