Fire audit report format in India: the official forms, field by field, and what a complete report contains
Updated By Flamekite Editorial DeskFacts checked against official sources on
Quick answer
No Act or rule Flamekite has read sets one national fire audit report format: each state that requires an audit prescribes its own. Maharashtra uses Form V with a "Fire, Safety and Audit report" attached, Telangana a 13-item certificate (2020), Chhattisgarh Form-Q with a detailed Fire Audit Report (2025), and Delhi a 20-head Form J inspection report from 20 May 2027.
Every official form ends the same way: one certificate per building, the auditor's signature, address and licence or registration number, and the report attached. For hospitals, the DGFS advisory checklist of the Home Ministry has 48 general items, then 23 to 64 more by building category.

How to build a fire audit report, part by part: Flamekite's 12-part structure
1. The certificate on your state's form
Put the prescribed certificate first: Form V in Maharashtra, Form-Q in Chhattisgarh, Form H in Delhi from 2027. Maharashtra and Chhattisgarh both require a separate certificate for each building.
2. Scope, law and code edition
State the Act, the rule and the code each finding is judged against. The forms were written against NBC 2016, which BIS withdrew on 30 April 2026 and replaced with SP 7:2026, so name the edition your state applies.
3. Building data
Name, address, height, the number of basements and upper floors, the occupancy at each level and the previous fire certificate: the first 6 items of Delhi's Form J.
4. Date of inspection and who was present
Delhi's Form H records the date of inspection and the owner's representative present; Form J records that person's name and designation.
5. Findings system by system
For each system, set what is required beside what is at site, as Form J's columns and Telangana's Required column do: numbers of extinguishers, litres in each tank, pump flows in LPM.
6. Each non-compliance with its clause
Tie every gap to the clause it breaks. Form J measures against the minimum standards of rule 33; the DGFS checklist gives a reference standard for each of its 48 general items.
7. A grade for each finding
The official forms grade in two values: MR or NMR, working or not working, Yes or No. Flamekite recommends adding a priority (life safety first, then protection, then paperwork); no rule sets one.
8. The tests performed
Record what was tested and how. Maharashtra's rule 33 requires the auditor to test all the active measures, not just inspect them.
9. Photographs and videos
Date- and time-stamped, and signed by the owner's representative, as rule 33(1)(xii) requires in Maharashtra.
10. Recommendations
A corrective measure for each gap, with who does it and by when. Telangana's G.O.Ms.No.15 of 2026 defines its auditors as those who identify hazards and recommend corrective measures under the NBC and BIS codes.
11. Annexures
Plans and certificates: Delhi's Form I asks for the site plan, the architect's height certificate and as-built plans; Telangana's Fire Audit module takes plans as PDFs under 3 MB.
12. The signature block
The auditor's name, address, licence or registration number, level, stamp and date, and the owner's countersignature where the form has one (Delhi Forms I and K, Chhattisgarh Form-R).
Maharashtra Form V: the fire audit certificate, word for word
Form V is a one-page certificate. The audit itself goes in the report it points to, which rule 33 makes the auditor sign and hand to the owner.
Word for word
It is hereby certified that I, M/s. ___ having license to carry out fire and life safety audit, have evaluated the fire prevention and life safety measure works in the following building or premises, as required under the provision of the Maharashtra Fire Prevention and Life Safety Measures Act, 2006 (Mah. III of 2007) and are found in good repair and efficient working condition.
The form then asks for the description and location of the building, says the details are "mentioned in the Fire, Safety and Audit report appended herewith", and closes with a declaration that the auditor has read the duties, the place, the date, the signature and address, and the licence number. Its note reads: "For each building a separate certificate in Form 'V' shall be necessary." Rule 31 lets the certificate go in person, by registered post, by courier or online. The gazette heads Form V "See rule 32", although rule 31 is the rule that creates it.
Maharashtra rule 33: the 12 auditor duties as fire audit report chapters
Flamekite’s working from the official rules; no government publishes this figure.
Rule 33 lists 12 duties of a licensed Fire and Life Safety Auditor. The duties are the rule's; turning each into a chapter of the report is Flamekite's reading.
(i) 360-degree assessment
Chapter: the fire hazard risk assessment of the whole building, inside and around it. Amendment Rules 2025
(ii) Construction
Chapter: the building as built against the fire safety regulations, floor by floor. Amendment Rules 2025
(iii) Ignition sources
Chapter: where flammable materials are stored, processed or moved, and how ignition is controlled there. Amendment Rules 2025

(vii) Building Management System or fire control room
Chapter: how the control room or BMS works in practice. Amendment Rules 2025
(viii) Staff readiness
Chapter: staff safety awareness, the training they need and how ready they are to handle an incident. Amendment Rules 2025
(ix) Report and Form V
The signed Fire Audit Report and Form V go to the owner or occupier; for a firm, the person holding the qualification signs. Amendment Rules 2025
(x) Working condition
Chapter: confirmation that every active and passive system is in good and efficient condition. Amendment Rules 2025
(xi) Tests
Chapter: the test of every active fire and life safety measure, with results. Amendment Rules 2025
(xii) Photographs and videos
Annexure: inspection and test images with date and time stamps, signed by the owner's representative. Amendment Rules 2025

Chhattisgarh Forms Q and R: a detailed fire audit report and a 30% re-check
Chhattisgarh's rule 37D, notified on 24 November 2025, is the only rule Flamekite found that names the report itself: a "detailed Fire Audit Report" attached to the auditor's certificate.
Rule 37D(2)(a)
Form-Q: the auditor's certificate
The licensed Third-Party Auditor certifies the work done to comply with the fire prevention and life safety audit, lists it in an appended schedule and signs with a licence number. Its notes: a detailed Fire Audit Report is attached, and each building needs its own certificate. CG Gazette 888
Rule 37D(2)(b)
Form-R: every January and July
A six-monthly certificate that the building was kept in good repair during a stated period, with the audit details in an appended report. It carries two signatures, the owner's or occupier's and the auditor's, and the owner files a self-attested copy. CG Gazette 888
Rule 37D(3)
A second audit by the department
An owner not satisfied with the third-party audit may pay the charges and ask the department to audit the premises through a qualified officer. CG Gazette 888
Rule 37D(4)
At least 30% re-checked
The Director General authorises qualified officers to verify at random at least 30% of the buildings third-party auditors have audited, and to act on major deviations. Read from the gazette images in Hindi and English. CG Gazette 888
Rule 37A(2)(b)
No certificate without compliance
No auditor may certify unless the building actually complies with the NBC, BIS codes or the approved Chhattisgarh building bye-laws, so a report with open gaps cannot end in a clean Form-Q. CG Gazette 888
Delhi Form J: the 20 heads of the fire audit inspection report
Delhi's Form J, in force from 20 May 2027, is the most detailed official audit report grid in India: 20 heads, each with the requirement, the status at site and a remark of MR (meeting the requirement) or NMR (not meeting it).

Hospital fire safety audit report format: the DGFS checklist by category
The Home Ministry's fire directorate (DGFS) issued an advisory checklist for auditing hospitals and nursing homes, file F. No. VI-14022/09/2024-DGCD (F). It sorts buildings into 4 categories and lists items with 5 columns: serial number, checklist item, reference standard, Yes or No, comments.
| Applies to | Items | Examples, with the reference the checklist gives | |
|---|---|---|---|
| General measures DGFS hospital checklist | Every hospital and nursing home | 48 | Last fire safety audit date (NDMA Guidelines 2016); extinguishers (IS 2190:2024); two exits a floor, 30 m travel, dead ends of 6 m at most (NBC 2016 Part 4, para 4.4) |
| Category A DGFS hospital checklist | Below 15 m, plot up to 1,000 m² | 23 | First-aid hose reel (IS 884:1985); no flammable partitions or curtains; oxygen cylinders stored safely |
| Category B DGFS hospital checklist | Below 15 m, plot over 1,000 m² | 31 | Wet riser and yard hydrants; manual call points; a public address system; gas storage as PESO requires |
| Category C DGFS hospital checklist | 15 m to 24 m | 37 | Emergency lighting; illuminated exit signs; fire compartmentation and fire dampers in central air conditioning (NBC 2016 Part 4); a fire control room |
| Category D DGFS hospital checklist | Above 24 m to 45 m | 64 | Thermographic or electric audits; ICU air handling over 10 air changes an hour; fixed oxygen leak detection (PESO) |
| Above 45 m DGFS hospital checklist | Hospitals taller than 45 m | None | No category in the advisory |
The checklist lists among its references a MoHFW circular on fire safety audits of 3 January 2020 and the NDMA hospital fire safety guidelines of 2016. Its closing note calls it a general guideline for state fire services, which may continue their own practice. The Health Ministry's 2026 national guidelines on fire safety in healthcare facilities were not found on an official site, so nothing from them is used here. In Telangana, hospitals above 15 m file the third-party certificate every January and July.
What no official fire audit report format publishes
Several forms are named in law but not printed where Flamekite could read them, and no form sets a risk grade. These 7 gaps are open on 25 October 2026.
Odisha's Second Amendment Rules of 4 November 2025 require audit certificates on Forms IV and IV(B) each January and July, but do not print the forms; they sit in the 2017 Rules, whose official gazette copy could not be opened. Madhya Pradesh's rule 87(5) puts its audit report in Form-S of Appendix-L, which the English copy of the Bhumi Vikas Rules read here leaves out.
Telangana's G.O.Ms.No.15 of 20 January 2026 adds an owner's January and July certificate as Annexure IV, but the published file has 2 pages and no annexures. No official form grades risk beyond two values, so a priority scale in a report is the auditor's own. No form fixes how long a report may run or a deadline for fixing its findings.

Fire audit report findings written out: 5 worked examples
Flamekite’s working from the official rules; no government publishes this figure.
How a finding should read in the report, using each state's own form. The buildings and readings are Flamekite's illustrations; the requirements are the forms'.
Case 1
Emergency lighting in an 18 m hospital in Hyderabad
Batteries held the lights for 60 minutes in the test.
What applies Row 12 of Telangana's certificate asks for at least 90 minutes, so the remark is "Not working", with the reading, the floors affected and the fix. A hospital above 15 m files every January and July. TG fire circular 2020
Case 2
Jockey pump in a 40 m apartment block in Hyderabad
The jockey pump's nameplate reads 150 LPM.
What applies Row 10 lists jockey pumps of 180 LPM, so the report records the 150 LPM found and marks the row. A residential building above 30 m and below 45 m files once a year, in January. TG fire circular 2020
Case 3
A 9 m dead-end corridor in a 20 m nursing home
Category C under the DGFS checklist.
What applies General measure 15 allows dead ends of 6 m at most (NBC 2016 Part 4, para 4.4), so the item is marked No, with the clause and a comment giving the length and the corridor. DGFS hospital checklist
Case 4
Missing fire check doors in a 12 m Delhi office, after May 2027
Two floors have no fire check door at the staircase.
What applies Form J head 2 (exits) is marked NMR. The auditor cannot then sign Form H's statement that the building complies with rule 33; a falsified form can cost a Level-1 auditor Rs 2 lakh. DFS
Case 5
Test photographs from a Maharashtra section 45 building
Pump tests photographed without a time stamp or any countersignature.
What applies Rule 33(1)(xii) wants date and time stamps and the signature of the owner's representative, so the tests are photographed again before Form V is signed. Amendment Rules 2025

Photographs and evidence in a fire audit report: dos and don'ts
Only Maharashtra's rule 33 sets photo rules for an audit. Its Form B guidelines of 7 July 2026 add a photo list for maintenance certificates that also suits an audit report.
Do
- Time- and date-stamp every inspection and test photograph or video, and have the owner's representative sign them (rule 33(1)(xii)). Amendment Rules 2025
- Photograph each fire pump running, with its pressure gauge read while the topmost hydrant or sprinkler test valve is open, as the 2026 Form B guidelines ask. MFS Form B guidelines
- Show the alarm panel, talkback, hose reel, booster pump, egress and refuge areas, and the changeover to the alternate power supply. MFS Form B guidelines
- Name the owner's representative present at the inspection, as Delhi's Form J does in its sixth item. DFS
Don’t
- Do not put several buildings on one certificate: Maharashtra's Form V and Chhattisgarh's Form-Q each need a separate certificate per building. CG Gazette 888
- Do not add words to or delete words from a prescribed form, or put the fire service's logo on it: the 2026 Form B guidelines bar both. MFS Form B guidelines
- Do not audit a building your firm designed, installed or maintains: Delhi's rule 35(7) bars it from 2027. DFS
- Do not leave pump readings unrecorded: Telangana's form asks for pump ratings in LPM and tank capacities in litres. TG fire circular 2020
Errors in a fire audit report that get it questioned, and the fix
Each of these 6 errors breaks a line in a published form or rule.
If A firm signs the report, but not the person who holds the licence qualification
Rule 33(1)(ix) of Maharashtra's rules has the qualified person of the firm sign the report and Form V. Amendment Rules 2025
If The same Delhi auditor signs three certifications in a row
From 2027 an auditor is barred from the next 2 consecutive certifications for a building; Form H's condition 7 sends Form K to another auditor. DFS
If Form I or Form K filed without the owner's countersignature
Both Delhi forms carry an owner's undertaking and signature; the owner answers under section 37 of the Delhi Fire Service Act 2007. DFS
If An Odisha high-rise audited by an ordinary empanelled auditor
Non-residential buildings above 15 m, homes above 30 m and premises needing 75,000 litres of underground water need an auditor with the rule 9(15)(a)(i) or (ii) certificate. Odisha Home Dept
If Findings that cite "NBC" without the edition
NBC 2016 was withdrawn on 30 April 2026; name the code edition your state's law applies, beside each clause. Gazette of India
If A Telangana certificate without a licence number or the building's address
The 2020 form asks for the auditor's licence number and a detailed location and postal address. TG fire circular 2020

Questions about the fire audit report format
What is the format of a fire audit report in India?
- It is the form your state prescribes: Form V with an appended audit report in Maharashtra, a 13-row certificate in Telangana, Form-Q with a detailed Fire Audit Report in Chhattisgarh, and Forms J, H and I in Delhi from 20 May 2027. No single national format exists in any Act or rule Flamekite read.
Is there an official fire audit report format PDF?
- Yes, in the states that prescribe one: the forms are printed in Maharashtra's Amendment Rules 2025, Telangana's circular of 28 January 2020, Chhattisgarh's gazette of 25 November 2025 and Delhi's Amendment Rules 2025. Flamekite links those files rather than offering its own template.
What should a fire safety audit report contain?
- At least the state's certificate, the building's data, findings system by system against the code, each gap with its clause, the tests done, time-stamped photographs, recommendations and the auditor's licence number and signature. Maharashtra's rule 33 sets 12 duties the report must reflect.
Who signs a fire audit report?
- The licensed or empanelled auditor, with a licence or registration number on every official form. Delhi adds the auditor's level, a stamp and a UID number, and has the owner countersign Forms I and K; Chhattisgarh's Form-R is signed by both owner and auditor.
What is the fire safety audit report format for a hospital?
- The state's form, worked against the DGFS advisory checklist for hospitals and nursing homes: 48 general items, then 23 to 64 more by height and plot, each marked Yes or No against a named standard. In Telangana, hospitals above 15 m file every January and July.
What is Form V in a Maharashtra fire audit?
- The certificate a licensed Fire and Life Safety Auditor signs after the section 45A audit, stating the measures are in good repair and efficient working condition. The Fire, Safety and Audit report is appended, and each building needs its own Form V.
Are photographs required in a fire audit report?
- In Maharashtra, yes: rule 33(1)(xii) requires photographs or videos of the inspection and tests with date and time stamps, signed by the owner's representative. Other states' forms do not mention photographs.
