Form B (fire) submission in Maharashtra: where to file, how, and the last date

Updated By Flamekite Editorial DeskFacts checked against official sources on

Quick answer

Form B fire submission in Maharashtra: file in January or July with your Chief Fire Officer or the Director; MIDC plots online only, in a 3-month window. The Act names the months, not a day, so Flamekite reads 31 January and 31 July as the last dates; rule 4(5) lets you hand it in, post it, courier it or file it online.

Mumbai's current route is not published, Pune has taken it at the fire office or by e-mail, and a Form B signed by an agency whose licence lapsed is accepted only within 3 months of the expiry. A missed filing can be prosecuted under section 36 of the 2006 Act.

How to submit a Form B, from the agency's certificate to the acknowledgement

  1. Find the office that receives your Form B

Use the table above: the MIDC SWC portal for an MIDC plot, your municipal Chief Fire Officer where there is one, and the Director, Maharashtra Fire and Emergency Services, where there is none.

  2. Check the agency's licence dates against the filing date

Read the licence validity on the agency's licence copy. If it expired more than 3 months before you file, the fire service need not accept the Form B: get a renewed licence or another agency first.

  3. Get a Form B dated in January or July with the right period

January filings state 1 July to 31 December of the previous year; July filings state 1 January to 30 June of the same year.

  4. Address the covering letter to the right office

Date it "Jan" or "July", address it to the Director at Kalina, Mumbai 400 098, or to your Chief Fire Officer by name and address, and quote the Final and Provisional approval numbers.

  5. Send it by a rule 4(5) mode inside the month

Hand it in, use registered post or a courier, or file online where your office runs a channel. MIDC plot holders upload it on SWC within 3 months of 1 January or 1 July.

  6. Keep proof of the date you filed

Keep the stamped copy, the postal or courier receipt, the sent e-mail or the SWC application record with the whole file you sent.

  7. Diary the next window and the agency's expiry

The next Form B falls due in the following July or January; agency licences run 2 years, so note the expiry date alongside.

Flamekite’s working from the official rules; no government publishes this figure.

The Act fixes the months, not a day. Flamekite's reading is that a Form B is on time if it is filed by the last day of January or July; MIDC's online window runs longer. Dates marked "about" are Flamekite's count.

5 ways a Form B submission reaches the fire service

Rule 4(5), added on 12 June 2025, names 4 modes; Pune's fire brigade has added e-mail. Which one an office accepts online is its own choice.

  • Rule 4(5)

By hand at the fire office

The owner or occupier hands the file to the Chief Fire Officer's office. MIDC plot holders did this until 1 January 2025, when MIDC moved the filing online. Amendment Rules 2025

  • Rule 4(5)

    By registered post

Sent to the Director or the Chief Fire Officer; the postal receipt dates the filing. Amendment Rules 2025

  • Rule 4(5)

    Through a courier agency

Named in the rule beside registered post, for the same offices. Amendment Rules 2025

  • Rule 4(5)

    Online

Allowed since 12 June 2025, but only MIDC has published a dedicated Form B service: the SWC portal's Bi-Annual Submission of Form-B. MIDC SWC

  • Pune, as reported

    By e-mail

PMC's appeal of 29 September 2025 was reported to accept the compliance certificate at the office or by e-mail; a 2024 report quoted the Chief Fire Officer: "mail it to us". Pune Pulsenews report

MIDC SWC Form B submission, screen by screen

MIDC's public form view shows every field of its Form B service; submitting needs a registered login, which Flamekite did not use. The service runs for 3 months from 1 January and 1 July.

  1. 1. Register the plot on SWC

Name, role (owner, company employee or consultant) and a Letter of Authority on company letterhead, as PDF, JPG or JPEG up to 3 MB, then the plot's area, industrial area and number. MIDC SWC

  2. 2. Prove an online Final Fire NOC

Only plot holders whose Final Fire NOC, or its renewal, was approved through SWC may file. A paper NOC means applying online for renewal of the Final Fire NOC first (paragraph 4 of circular P-379286). MIDC circular

  3. 3. Choose the licensed agency

Pick it from the drop-down and enter its licence number and the date its licence is valid up to. If the agency is missing or wrong, MIDC says to contact its headquarters fire department. MIDC SWC

  4. 4. State the building

The approved built-up area as per the Final Fire NOC, in m², and whether an occupancy certificate was obtained. MIDC SWC

  5. 5. Upload the 8 documents

7 mandatory and 1 optional, listed in the next section. MIDC SWC

  6. 6. Sign the declaration

The applicant declares the information true and accepts punishment as per law, and withdrawal of benefits, if it proves false. MIDC SWC

  7. 7. Clear MIDC dues first

The form carries alerts that SWC services stay closed until the plot's consumer number is updated or an outstanding balance is paid. MIDC SWC

The 8 uploads on MIDC's Form B submission screen

MIDC's circular lists 6 mandatory documents; the SWC screen splits them into 8 rows, 7 of them mandatory.

  • 1. Form B of the licence agency with its Annexure, as the 2006 Act provides (mandatory). MIDC SWC
  • 2. Annexure detailing the active and passive fire protection installed, with seal and digital signature (not mandatory). MIDC SWC
  • 3. The agency's valid licence copy issued by the Directorate of Maharashtra Fire Services, with a valid digital signature (mandatory). MIDC circular
  • 4. Covering letter on the owner's or occupier's letterhead (mandatory). MIDC SWC
  • 5. Appendix or Annexure with Form B for each active and passive system on the premises (mandatory). MIDC SWC
  • 6. Order copy of the AMC or CAMC awarded to the licensed agency (mandatory). MIDC SWC
  • 7. Testing report for the last month, quarter or half year, signed by the owner or occupier and the agency (mandatory). MIDC SWC
  • 8. Last site inspection report signed and sealed by the agency (mandatory). MIDC SWC

Form B submission offices: the Director, Pune, Nagpur and MIDC

Only these offices publish an address or channel for Form B; the Pune entries come from news reports of the brigade's appeal, which disagree on the office.

The 3-month licence grace for Form B submission, in MFS's words

A Form B is only as good as the agency's licence on the day it is given. MFS allows one margin, in a note to clause 2 of its July 2026 guidelines.

Word for word

(Note: Form A or B may be accepted by local fire and emergency service, if not more than 3 months has passed after the expiry of the license)

Guidelines for Form B, Annexure A, clause 2, circular MFS-2026/888 of 7 July 2026, Maharashtra Fire and Emergency Services MFS Form B guidelines

The note says "may": acceptance inside the 3 months is the fire service's choice, not a right. An agency whose licence expired on 30 September 2026 is inside the margin until 30 December 2026, so its January 2027 Form B falls outside it. Clause 4 governs a change of agency: either side gives 1 month's notice before work stops, the new agency obtains the old one's no-objection, and if that fails the Director may authorise another agency under section 10 without it.

Form B submission problems with the agency or the portal, and the fix

These are 6 reasons a Form B cannot be filed on time that the circulars and the MIDC screen anticipate.

  • If The agency's licence expired more than 3 months before filing

The 3-month note no longer helps. File with an agency holding a current licence; check its validity on the MFS licence pages first. MFS Form B guidelines

  • If The agency stops work weeks before January or July

The contract should give 1 month's notice. If the new agency cannot get a no-objection, ask the Director to authorise it under section 10. MFS Form B guidelines

  • If The MIDC drop-down does not list your agency

MIDC's note on the screen: contact the MIDC headquarters fire department to have the agency added or corrected. MIDC SWC

  • If An MIDC plot's Final Fire NOC was issued on paper

Apply online for renewal of the Final Fire NOC to register on SWC; only then does the Form B service open to you. MIDC circular

  • If SWC refuses the login over MIDC dues

The screen withholds SWC services until the consumer number is updated or the outstanding balance is paid; clear it before the window closes. MIDC SWC

  • If A report says Form B is due in January and June

A June 2024 news report said so; section 3(3) says January and July. Pune Pulsenews report

Late or missed Form B submission: what the Act and the offices can do

The Act sets no late fee and no grace for the owner. A missing Form B is an offence; the amounts below were proof-read against the gazette page.

Mumbai trade: Form A or B missing when a health licence is renewedMCGM may suspend or revoke the licence after inspection, under the undertaking on Rs 500 stamp paperUndertaking cum indemnity bond, clauses 6 and 11 MCGM

No court decision on a missed Form B was found. Reading non-filing as section 36(2)(c) is Flamekite's, not a court's or MFS's. Disconnection of water or power is a section 8 power for imminent danger, not a penalty for filing late.

What happens after a Form B is submitted

Nothing is issued back: the Rules prescribe no acknowledgement or approval of a Form B. It stands until the next maintenance or the next Form B, and the fire service may test it by inspection.

MFS's 2026 guidelines say a Form B holds "till the next schedule maintenance of equipment as per relevant Indian Standard and / or till the next Form B is issued". The next one is due in the following January or July, whatever the date this one was filed.

Filing does not shield the building from inspection. MFS's circular of 31 January 2025 tells a fire officer who doubts a building to inspect it under section 5 and serve a notice under section 6, rather than ask for a renewal. Where another authority demands a renewal, MIDC's 2015 circular, repeated on 1 January 2025, has the fire officer inspect and send the report with a copy of the Form B to the owner for that authority.

The filed Form B is evidence for other papers. MIDC's fire NOC procedure asks an existing factory for its Form B with Annexure, and Mumbai's undertaking for renewing a health licence has the trader state that Form A or Form B was obtained from a licensed agency.

What the MIDC screen shows after submission, and whether a Divisional Fire Officer reviews each Form B, is behind the login and not published.

Questions about Form B submission

What is the last date for Form B submission?

The Act says only "in the months of January and July", so no day is fixed. Flamekite reads the last dates as 31 January and 31 July. On MIDC plots the online service stays open for 3 months from 1 January and 1 July.

How do I submit Form B online?

On an MIDC plot, register on the MIDC SWC portal and use Bi-Annual Submission of Form-B, with 8 uploads. Elsewhere, rule 4(5) allows online filing, but no other office has published a Form B service; Pune has taken it by e-mail, as reported.

Where do I submit Form B in Mumbai?

To the Mumbai Fire Brigade, but its current route is not published. Its only public notice is an undated draft appeal that asked high-risk buildings to upload Form B on the MCGM site by January 2016. Send it by registered post or courier and keep the receipt.

Where is Form B submitted in Pune?

To the Chief Fire Officer of the Pune Fire Brigade, at the fire office or by e-mail, as PMC's September 2025 appeal was reported. Two reports name different offices on New Timber Market, so confirm with the brigade.

Who submits Form B, the agency or the society?

The owner or occupier, such as the society's chairman or secretary. The licensed agency signs the Form B; the owner files it with a covering letter and declaration.

Can I submit Form B if my agency's licence has expired?

Only within 3 months of the expiry, and only if the fire service agrees: MFS says Form A or B "may be accepted" in that time. After it, use an agency with a current licence.

What happens if Form B is submitted late?

The Act sets no late fee. A missing Form B can be prosecuted under section 36; Flamekite reads it as section 36(2)(c), with rigorous imprisonment of 6 months to 3 years and a fine. Pune's brigade warned that a non-filer bears sole responsibility for any mishap.

Is there a fee to submit Form B?

Flamekite found none in rule 4 or the MFS and MIDC circulars. The owner pays the agency's maintenance contract, which is a private price.
