Form A (fire) in Maharashtra: the licensed agency's installation certificate, and Form A-1

Updated By Flamekite Editorial DeskFacts checked against official sources on

Quick answer

Form A is the certificate a Maharashtra Licensed Agency signs once, when it has installed a building's fire prevention and life safety measures, stating that it executed the works the Act requires. It is prescribed by rule 4(1) of the 2009 Rules under section 3(3) of the 2006 Act, one certificate per building, with a list of the works appended, and it goes into the file for the Final Fire Safety Approval.

Form A-1, added on 12 June 2025, is the same kind of certificate for the automated continuous monitoring system that section 45 buildings, such as homes above 70 m, must fit. Since 2025 both are signed by a person holding the rule 12 qualification, digitally, with the signature on the agency's licence. A false Form A carries 6 months to 3 years of rigorous imprisonment.

Getting a Form A and Form A-1, from installation to the Final approval

  1. Hire a Licensed Agency for each system

Check the agency on the MFS list valid to 03.07.2028 in the column for the work: fire fighting, detection or passive. A class held in one system does not cover another.

  2. Install to the Provisional approval and the codes

Since 12 June 2025 Form A certifies compliance with the Act, the National Building Code and the Indian Standard Code, so the works follow the recommendations of the Provisional Fire Safety Approval and those codes.

  3. Witness the tests and keep the reports

Pump and pressure tests come before the certificate. PMRDA, for one, asks for a pump test certificate, a pressure certificate with the Form A and at least 10 photographs of the installation.

  4. Collect Form A with its list of works

One certificate per building, with the appended list, digitally signed by the agency's rule 12-qualified signatory with the signature on its licence, and its licence number.

  5. Add Form A-1 in a section 45 building

Homes above 70 m, hotels, hospitals and offices above 30 m and the large industrial and oil and gas sites need the monitoring system fitted and certified on a separate Form A-1.

  6. File it with the Final approval application

Submit to the Chief Fire Officer or the Director with the other final papers, in person, by registered post, by courier or online. Keep a copy: Form B takes over every January and July after occupation.

The words of Form A, read off the 23 June 2009 gazette

Form A is a one-page certificate in the Rules of 23 June 2009. The agency certifies works it executed; it has no period, no owner's signature and no fee.

Word for word

Certified that I/We have executed the works towards compliance in relation to Fire Prevention and Life Safety Measures to be provided and performed other related activities required to be carried out, in the following building or premises, as required under the provisions of the Maharashtra Fire Prevention and Life Safety Measures Act, 2006 (Mah. III of 2007).

Form A, "[See section 3(3) and rule 4(1)]", Maharashtra Fire Prevention and Life Safety Measures Rules, 2009, gazette of 23 June 2009 Rules 2009

Below the statement come the description and location of the building, the line "The details of the work and related activities which I or we have executed or performed are mentioned in the list appended herewith", the place and date, the signature and address of the Licensed Agency and its licence number. A footnote reads: "For each building a separate certificate in Form 'A' should be necessary." Rule 4(1) ties it to "a building or a part thereof", and the 2025 amendment added the National Building Code and the Indian Standard Code to what it certifies. The Act itself never names Form A: section 3(3) asks only for a certificate "in the prescribed form".

What Form A-1 certifies: the 20 signals of the monitoring system

Form A-1 certifies that the agency "executed the works as mentioned in Appendix". The Appendix of 12 June 2025 lists 20 parameters across 8 systems that an IoT gateway must report to the owner and the agency.

  1. 1. Hydrant pumps and power

Run status of the main hydrant, standby diesel or electric, jockey and booster pumps; power to the main hydrant pump; diesel tank level; hydrant riser low pressure. Amendment Rules 2025

  2. 2. Fire water tanks

  4. 4. Detection

Detection on or off, the control or repeater panel working, and the panel battery, through contacts taken from the fire panel. Amendment Rules 2025

  5. 5. Manual call points

Operation status of the manual call points. Amendment Rules 2025

  6. 6. Public address

Working or not, from the PA panel's fault status. Amendment Rules 2025

  8. 8. Basement fans

Supply and exhaust fans for basement ventilation running. Amendment Rules 2025

Papers that go with a Form A into the Final Fire Safety Approval file

The Rules do not list the papers for a Final approval; the authorities' own lists do, and MFS, the Mumbai Fire Brigade and PMRDA all ask for the agency's certificate.

  • MFS, for approvals from the Director: item 6 of 6, a certificate issued by a Licence Agency approved by the Director or the Chief Fire Officer; the fire service may also inspect and check the Provisional NOC's recommendations. MFS e-fire
  • Mumbai Fire Brigade, occupation NOC: "Form A with its annexure" from a licensed agency for all the fire installations it stipulated, with a site inspection, structural stability and, at 24 m or more, PWD lift certificates. Mumbai Fire Brigade
  • PMRDA, Final Fire NOC: item 5, Form A with its list, stamped and signed; item 6, the agency's licence copy with digital signature. PMRDA fire

Who may sign a Form A since 12 June 2025

Rule 14(4) says Forms A, B, A-1 and B-1 are issued "under signatory of the person having requisite qualification as mentioned in rule 12". There are 2 qualification routes and 2 ways a firm meets them.

  • Route (a)

    A fire qualification

A degree, advanced diploma or diploma in fire engineering from the National Fire Service College, a UK fire institution membership or graduateship named in rule 12, or since 2025 a degree from a UGC or AICTE-recognised body. Amendment Rules 2025

  • Route (b)

    Engineering and 3 years

A degree or diploma in mechanical, electrical, electronics, chemical or civil engineering or computer application, plus at least 3 years executing fire systems to the NBC or NFPA, with an officer holding a route (a) qualification employed. Amendment Rules 2025

  • Proforma C

8 Form A problems that hold up a Final Fire Safety Approval

  • If A Form A on letterhead with a rubber stamp and a scanned signature

Since 15 February 2022 MFS requires Form A and its annexure to carry the agency's valid digital signature, the same as on its licence. The rule followed complaints of fake certificates. MFS circular 236

  • If No list of works attached

  • If The agency's licence lapsed before it signed

The fire service may still accept its Form A or Form B if not more than 3 months have passed since expiry. Beyond that, the agency renews or you use another. MFS Form B guidelines

  • If The agency holds detection but signed for the hydrants

Licences are granted per system and class. Find the licence number in the fire fighting column of the MFS list before accepting the certificate. MFS agency list

  • If The signatory has no fire or engineering qualification

  • If A tower above 70 m files a plain Form A

Section 45 buildings also need the monitoring system and its Form A-1. Amendment Rules 2025

  • If The agency is on the blacklist

Form A worked cases: 6 Maharashtra jobs

Flamekite’s working from the official rules; no government publishes this figure.

Flamekite's own reading of the Act, the Rules and the authorities' lists, applied to typical jobs.

  1. Case 1

    A 68 m residential tower in Thane

New, fire works complete, Final approval not yet applied for.

What applies Form A from each installing agency, one per building. At 68 m it is under the 70 m line, so no Form A-1. Amendment Act 2023

  2. Case 2

    A 74 m residential tower in Mumbai

    Above 70 m, so a section 45 building.

What applies Form A and Form A-1, filed with the occupation papers, which in Mumbai include "Form A with its annexure" and a site inspection. Section 45 also asks for a fire officer or fire supervisor. Mumbai Fire Brigade

  3. Case 3

A 32 m hospital near Pune in PMRDA limits

    A hospital above 30 m, so a section 45 building.

What applies Form A with its list and Form A-1, the agency's licence copy with digital signature, its site report with the Form-A number, pump and pressure certificates and at least 10 photographs. PMRDA fire

  4. Case 4

A heritage college building in Nashik retrofitting detection

Cabling is restricted, so wireless detectors are chosen.

What applies Allowed under clause 4.9 of IS 2189 by the circular of 16 October 2026, fitted by a licensed agency that issues Form A for the installation and Form B for its upkeep. MFS circular 1420

  5. Case 5

A mall whose agency's licence expired on 10 March 2026

Form A signed on 2 May 2026, 1 month and 22 days later.

What applies Within the 3-month grace in the MFS guidelines, so the fire service may accept it. A Form A signed after 10 June 2026 would not be. MFS Form B guidelines

  6. Case 6

A Mumbai restaurant renewing its health licence

Checking a Form A before the owner files it

The owner files Form A with the fire officer, so 5 checks come first.

Do

  • Find the agency's licence number on the MFS list valid to 03.07.2028, in the column for the system it certifies. MFS agency list
  • Open the digital signature on Form A and its annexure and compare it with the agency's licence. MFS circular 236
  • Check that the building's description and location are filled in, one certificate per building. Rules 2009
  • Keep the pump and pressure test certificates the authority asks for with the Form A. PMRDA fire
  • Confirm the licence was valid when Form A was signed, or expired less than 3 months before. MFS Form B guidelines

Don’t

  • Don't accept a Form A for a section 45 building without Form A-1 for the monitoring system. Amendment Rules 2025
  • Don't accept a certificate signed by someone the agency has not named as its rule 12-qualified signatory. MFS Form M
  • Don't let an installer outside the Licensed Agency list do the work: that is itself an offence under section 36(1)(f)(i). MFS Act 2006 copy

Where the Form A rules, lists and checklists are online

Every document behind this page is on 4 official sites. There is no separate online Form A service; rule 4(5) lets you submit online to your fire authority.

What Flamekite could not confirm about Form A and Form A-1

Seven gaps remained after reading the Act, both sets of Rules, the circulars and the authorities' lists on 25 October 2026.

  • How MFS applied section 3(3A) and Form A-1 to buildings already occupied when Mah. Act XXIV of 2023 came into force on 30 May 2023 (UDD notification FFS-2022/C.R.36/UD-14): no transition order is published.
  • When Phase II of the monitoring system, alarms to the fire service, starts: the 2025 Appendix says only "at later stage".
  • A prescribed format for the list of works appended to Form A: neither the 2009 Rules nor the MFS pages publish one.
  • Any prosecution or court order for a false Form A in Maharashtra: a search on 25 October 2026 found none.
  • Whether the Mumbai Fire Brigade takes Form A online today, as rule 4(5) has allowed since 12 June 2025.
  • Uttar Pradesh's "table E1", which Form A-5 cites for agency turnover, and the categories A to E on Form A-6: neither is in the 2024 gazette.
  • Uttar Pradesh rule 55(2) also names "Form A-4" for a Fire Safety Officer's compliance report; Flamekite reads it as a slip.

Questions about Form A (fire)

What is Form A fire certificate in Maharashtra?

It is the one-time certificate a Licensed Agency signs after installing a building's fire prevention and life safety measures, stating that it executed the works as the Act, the National Building Code and the Indian Standard Code require. Rule 4(1) of the 2009 Rules prescribes it, one per building.

Is Form A required for a fire NOC?

In practice, yes, at the final stage. The MFS E-Approval list for a Final NOC, the Mumbai Fire Brigade's occupation list and PMRDA's final checklist all ask for the licensed agency's certificate, and Mumbai and PMRDA name Form A with its list.

What is the difference between Form A and Form B in fire?

Form A is given once, when the works are installed, and certifies that they were executed. Form B is given every January and July for as long as the building is occupied, and certifies that the installations were maintained over the past 6 months.

What is Form A-1 in fire?

Form A-1 is the Licensed Agency's certificate that the automated continuous monitoring system has been fitted in a section 45 building, such as a residential tower above 70 m. It was added by the Amendment Rules of 12 June 2025 under rule 4(3).

Who can issue Form A?

Only a Licensed Agency licensed by the Director of Maharashtra Fire and Emergency Services, through a signatory holding the rule 12 qualification, with the digital signature on its licence. The Director may authorise another person under section 10 when no Licensed Agency can do the work.
